- Millcreek Home Health and Hospice Executive Director Nan Green, Physical Therapist (PT)
As the Executive Director, Ms. Green oversees operations, quality assurance, and compliance to standards of care in all disciplines. She participates in care management through hands-on involvement from admission to discharge, including post-service patient contact. Her commitment to excellence is evident in the extensive quality assurance processes with a strong focus on agency-wide education at Millcreek Home Health and Hospice. Ms. Green has 27 years of clinical experience ranging from orthopedic out-patient to in-home rehabilitation of neurological, orthopedic and cognitively impaired patients.- Millcreek Home Health and Hospice Chief Medical Director Danesh Rahimi, M.D.
Dr. Rahimi is Board-Certified in Internal Medicine and Emergency Medicine and is currently becoming Hospice Certified by the American Academy of Hospice and Palliative Medicine. He is also Board-Eligible in Geriatrics. His extensive history and education, coupled with his comprehensive understanding of geriatric medicine, make him the ideal candidate to direct medical care for Millcreek Home Health and Hospice. Dr. Rahimi is a strong believer in preventative medicine, palliative care and health education. Dr. Rahimi’s expertise leads Millcreek Home Health and Hospice’s team in specializing in symptom management, end of life care, pain control and treatment in a home environment. Dr. Rahimi graduated from the University of Utah Medical School in 1985, and completed his residency in Internal Medicine and his fellowship in Geriatrics at the University of Connecticut.- Millcreek Home Health and Hospice Medical Director Dr. Alexander Del Castillo, M.D.
Dr. Alexander Del Castillo brings a strong history of providing exceptional senior care to Millcreek. He earned his medical degree from University of Santo Tomas and completed his internship and residency at Metropolitan Hospital and New York Medical College. Dr. Del Castillo completed a fellowship at Shands Hospital at the University of Florida. He is board certified in internal medicine and geriatric medicine. With over 20 years of experience in medicine, Dr. Del Castillo helps Millcreek’s hospice team provide the highest quality care possible to our patients.- Millcreek Home Health and Hospice Medical Director Dr. Toby Zirkle, M.D.
Dr. Zirkle is a family medicine physician who specializes in diabetes and primary care. After receiving his medical degree from Indiana University Medial School in 2005, he moved to Utah where he completed a family medicine residency at St. Mark’s Hospital in 2008. Dr. Zirkle has a strong interest in diabetes management and has enjoyed participating in many pediatric diabetes camps, community health fairs, and workshops as well as performing in-patient diabetes consultations at St. Mark's Hospital. He also has volunteered for disaster relief events such as Hurricane Katrina, 9/11, and in a refugee camp during the war in Kosovo. His other medical interests include trackside medicine at international bobsled and skeleton competitions as well as ringside medicine working with amateur boxers, kick boxers and mixed martial artists. He is also the medical director of the Wilderness Adventure Society and an active research investigator in diabetes related clinical trials. On a personal side, he enjoys hiking, backpacking, mountain biking, martial arts, snow boarding and spending time with his wife and new daughter. - Millcreek Home Health and Hospice Director of Clinical Services Angel Sullivan, Registered Nurse (RN, BSN), Certified Hospice and Palliative Nurse (CHPN), Certified Lymphedema Therapist (CLT)
Ms. Sullivan holds certifications in both Lymphedema and Hospice and Palliative Care and holds a Master’s degree in Healthcare Administration with an emphasis in Gerontology. She has worked exclusively in the Home Health and Hospice since 2004. Her clinical expertise is critical in her role as a leader, educator and supervisor for nurses and aides at Millcreek Home Health and Hospice. In addition to being part of Millcreek’s management team, she continues to provide exemplary direct patient care.- Millcreek Home Health and Hospice Director of Therapies Rob Green, Physical Therapist (PT)
Mr. Green has 30-plus years of clinical experience with a strong focus on knee and shoulder rehabilitation. He provides focused leadership to a team of physical, occupational and speech therapists to ensure a comprehensive interdisciplinary team approach to care and rehabilitation at Millcreek Home Health and Hospice. Mr. Green directs regularly scheduled case conferences, greatly enhancing Millcreek’s inter-disciplinary approach to comprehensive patient care.- Millcreek Home Health and Hospice Patient Care Coordinator Vicki Tracy, Registered Nurse, (RN, BSN)
Ms. Tracy has the opportunity to work with nearly all of Millcreek's home health patients. She is charged with providing care coordination for all of Millcreek's new and returning patients. Vicki works to ensure that patients are admitted to home health in a timely manner and that they are matched with the right nurses and therapists for their individual needs. She is also one of Millcreek's bilingual nurses and provides high quality care to our Spanish speaking patients. Vicki earned her Bachelors of Science degree in Nursing from The University of Utah and has worked as a hospital nurse and home health case manager.
- Millcreek Home Health and Hospice Patient Care Coordinator Andrea Barnard, Registered Nurse, (RN, BSN)
Ms. Barnard works as a part of Millcreek's Patient Care Coordination Team, helping to ensure that each new or returning patient receives timely, high quality care. Andrea comes to Millcreek with a strong history of service to her community, travel and studying Spanish. Andrea earned her nursing degree from The University of Utah and began working with Millcreek as a Case Manager immediately after graduating.- Millcreek Home Health and Hospice Clinical Educator Maren McCauley, Registered Nurse (RN, BSN), Certified Hospice and Palliative Nurse (CHPN)
Ms. McCauley earned her degree in Nursing from the University of Utah and also holds a Bachelor’s degree in Economics from the University of Pennsylvania. She is a Certified Hospice and Palliative Care Nurse. Maren directs training and ongoing education for Millcreek Home Health and Hospice staff, participates in community outreach efforts, and continues to provide direct patient care. - Millcreek Home Health and Hospice Wound Care Specialist Peggy Riedesel, Physical Therapist (PT), Certified Wound Care Specialist (CWS)
Ms. Riedesel has been a practicing physical therapist for 25 years and has more than 9 years of wound care experience. She earned her certification as a Wound Specialist through the American Academy of Wound Management in 1999. Ms. Riedesel has extensive experience in acute out-patient and private practice settings, working closely with physicians, vascular and plastic surgeons, and other specialists addressing complex wounds. Ms. Riedesel is well respected and her services are in high demand throughout the local health care community. Her expert services allow Millcreek Home Health and Hospice’s patients to receive specialized wound management without the overly stressful effort of leaving home.- Millcreek Home Health and Hospice Chaplain John Crepeau, MA
Mr. Crepeau has more than 7 years experience in health care, social services, health education and pastoral care. As Chaplain at Millcreek Home Health and Hospice, John coordinates and provides spiritual care and emotional support to our patients, families and staff. He received a Master of Arts degree in Consciousness Studies from John F. Kennedy University where he studied human development from the perspectives of body, mind and spirit. Mr. Crepeau is currently enrolled in the Veteran’s Administrations Clinical Pastoral program through the College of Pastoral Supervision and Psychotherapy. - Millcreek Home Health and Hospice Medical Social Worker Heather Bittinger, Licensed Clinical Social Worker (LCSW)
Ms. Bittinger has more than 15 years of experience in the health care industry, in a variety of clinical capacities. She directs Millcreek Home Health and Hospice’s social services and bereavement program and is a critical part of Millcreek’s patient outreach services. Ms. Bittinger holds a master’s degree in social work from the University of Utah. As part of Millcreek Home Health and Hospice’s community outreach program, Ms. Bittinger is an active member of the Utah Social Work Leaders in Health Care and Utah State’s Commission for long term care planning.- Millcreek Home Health and Hospice Physical Therapist Craig Bischoff, Physical Therapist (DPT)
Mr. Bischoff has worked as a home health physical therapist since 2006. He earned his Doctorate of Physical Therapy degree from the University of Utah and immediately began working with seniors as a physical therapist in a skilled nursing facility. Craig brings a love of service and highly skilled care to Millcreek, which translates to great outcomes for his patients. When he isn't working, Craig loves to volunteer and run marathons.
- Millcreek Home Health and Hospice Physical Therapist Ben Gubler, Physical Therapist (DPT)
Mr. Gubler is a recent graduation from the University of Utah's Physical Therapy Program. He earned his Doctorate of Physical Therapy in 2009 and began working with Millcreek Home Health in 2010. Ben does an excellent job working with seniors to correct balance deficits, gait irregularities and numerous other problems that put seniors at risk for a fall. - Millcreek Home Health and Hospice Physical TherapistRebecca Sears, Physical Therapist,(PT)
Ms. Sears has worked as a physical therapist since 1998, when she earned her Master of Physical Therapy degree from the University of Utah. Rebecca has worked in numerous care settings including hospitals, skilled nursing facilities and rehabilitation centers. She brings her love of working with seniors to Millcreek and helps each of her patients retain their independence. When she isn't working, she loves to spend time exploring the outdoors with her daughter - Millcreek Home Health and Hospice Occupational Therapist Eric Dalling, Occupational Therapist (OT)
Mr. Dalling has worked in health care since 1994 and has over ten years experience as an Occupational Therapist. He has worked with patients in both rehab facilities and home care environments to successfully improve their comfort and safety as they perform activities of daily living. Mr. Dalling earned a Bachelor of Science in Exercise Physiology from the University of Utah and a Master of Science in Occupational Therapy from the Arizona School of Health Sciences.
